We welcome new and established patients to make appointments. Patients are seen by appointment at all of our practices, and walk-ins are welcome at our urgent care. We often offer same-day and next-day appointment availability at many of our locations, when needed. To schedule an appointment, please call the practice your provider is located.
Primary care new patient access line
Our team of physicians, nurse practitioners, and physician assistants provide expert primary care services for the entire family. Their compassionate medical care includes preventative care, chronic disease management, and access to specialists, when needed. New patients
We kindly ask that new patients arrive 15 minutes early to complete paperwork. You can download, print, and complete the forms prior to your appointment. Please bring the completed forms, a list of all medications you are currently taking, recent copies of X-rays, lab results and your I.D. and insurance card to your appointment.

Billing
Your co-payment and/or past-due account balance are due at the time of service. We accept checks, debit and all major credit cards. You can access your records and billing summaries via our patient portal.
You can also use our secure Online Bill Pay option to pay current statement balances online without setting up an account.
Insurance
We will submit claims to most major insurance carriers. Please bring your insurance card and I.D. with you to every visit so we can ensure that our records are accurate.
We recommend that you call your insurance company to verify that we participate with your plan.
Please notify us if you have any change in your name, phone number, address, or insurance as soon as it occurs. And please be prepared to present your insurance card at every visit.
Cancellations or rescheduling
To better serve the needs of all our patients, we ask that you call at least 24 hours in advance to cancel or reschedule appointments.
Refill requests
We are happy to help with refill requests during regular office hours. Please call the practice your provider is located or send a message via the patient portal.
