Outpatient Behavioral Health Transition to Community-Based Partners
Effective March 31, 2026, outpatient behavioral health services will transition to our trusted community-based partners to ensure continued access to care. Catholic Medical Center is committed to ensuring all impacted individuals experience a seamless transition, with clear communication, care coordination, and support to maintain continuity of care.
We have proactively reached out to 100% of our current patients to support a smooth transition. Each patient has received a letter and follow-up phone call to review next steps, discuss care options, and ensure continuity of services. Our team is actively monitoring call volumes and availability to provide timely assistance and address any questions or concerns.
We are working closely with local community providers who have the capacity and availability to continue care. The partners below are prepared to accept new patients and provide ongoing behavioral health services to meet individual needs.
